Form E111 for health care whilst travelling within the EU has been made obsloete since 2006
5-year plastic (credit card style) health care card is now issued by EHIC. Apply for a 5-year card here
If you fall ill or have an accident most of the costs of treatment and medication are recoverable when visiting EU member countries, but in order to qualify you will need to produce your EHIC card.
Please note that the EHIC card is not a substitute for comprehensive travel insurance and will not cover you for cancellations, lost luggage, personal liability etc.
